the above wazifa is the three names of ALLAH Ya-Wadudu Ya-Ra’ufu Ya-Rahimu
also as you mention take his/her name with mother name…is it suppose to be other persons mother name of yours? and what if you don’t know her mothers name? plz reply back thanks
